This article describes the most important determinant of contemporary American medical practice: the corporatization of the health care delivery system. It argues that there is an urgent need for greater reflection by physicians on the values inherent in profit-based health care and on the implications of such a model of care. Other pressures on the medical profession and several available responses are examined. While Michael Walzer's distinction between preemptive and preventive wars offers important categories for current reflection upon the Bush Doctrine and the invasion of Iraq, it is often treated as a modern distinction without antecedent in the classical Christian just war tradition. This paper argues to the contrary that within Augustine's corpus there are passages in which he speaks about the use of violence in situations that we would classify today as preemptive and preventive military action. While I do not claim (...) that Augustine makes an explicit distinction between the two types of war (such would be anachronistic), I will argue that based on examinations of "De libero arbitrio" I.v.11-12 and "De civitate Dei" I.30 Augustine's discussions of hypothetical cases or actual wars in history provide insights helpful for contemporary reflection on preemptive and preventive wars. The corporatization of U.S. health care has directed cost containment efforts toward scrutinizing the clinical decisions of physicians. This stimulated a variety of new utilization management interventions, particularly in hospital and managed care settings. Recent changes in fee-for-service medicine and physicians' traditional agency relationships with patients, purchasers, and insurers are examined here. New information systems monitoring of physician ordering behavior has already begun to impact on physician autonomy and the relationship of physicians to provider organizations in both for-profit and not-for-profit (...) sectors. As managed care practice settings proliferate, serious ethical questions will be raised about agency relationships with patients.This article examines health system dynamics altering the historical agency relationship between the physician and patient and eroding the traditional autonomy of the medical profession in the United States. The corporatization of medicine and the accompanying information systems monitoring of physician productivity is seen to account of such change, now posing serious ethical dilemmas. (shrink)

Since Aristotle, the concept of the magnanimous or great-souled man was employed by philosophers of antiquity to describe individuals who attained the highest degree of virtue. Greatness of soul was part of the language of Classical and Hellenistic virtue theory central to the education of Ambrose and Augustine. Yet as bishops they were conscious of fundamental differences between Christian and pagan visions of virtue. Greatness of soul could not be appropriated whole cloth. Instead, the great-souled man had to be baptized (...) to conform with Christian understandings of righteousness, compassion, and humility. In this book, J. Warren Smith traces the development of the ideal of the great-souled man from Plato and Aristotle to latter adaptions by Cicero, Seneca, and Plutarch. He then examines how Ambrose's and Augustine's theological commitments influenced their different critiques, appropriations, and modifications of the language of magnanimity. (shrink)

Les variables comprenaient le type de travail attendu, le nombre d’heures de travail prévu, les préoccupations liées à l’emploi, les mesures de soutien souhaitées sur le lieu de travail et l’influence de la gravité de la déficience intellectuelle et de l’expérience de travail du coordonnateur de services sur les attentes en matière de (...) travail. Les données ont été recueillies auprès de 46 personnes ayant une déficience intellectuelle par le biais d’entretiens et de 46 coordonnateurs au moyen d’un sondage en ligne. Les résultats indiquaient des attentes professionnelles positives globales. Le travail le plus courant attendu était le service et le commerce. L’emploi concurrentiel était attendu plus fréquemment que les ateliers protégés. Alors que les attentes des coordonnateurs de services et des personnes ayant une déficience intellectuelle légère étaient étroitement alignées, il existait une distinction plus grande entre les attentes des coordonnateurs et celles ayant une déficience intellectuelle modérée ou grave. Ces résultats ont des implications importantes pour faciliter le placement des adultes ayant une déficience intellectuelle. (shrink)

Each year, millions of individuals in the United States are treated for a variety of serious medical conditions with prescription drugs whose therapeutic benefits are well known. The vast majority of these medications are used to treat medical and psychiatric illnesses. Generally, they are used as prescribed, and contribute to a better quality of life for persons suffering from debilitating or life-threatening disorders.The fact that a small portion of these medications is diverted by those who seek their psychoactive effects raises (...) the important policy issue: how to make drugs easily available for medical use while limiting access for purposes of abuse.Such a responsibility poses challenges very different from those of the so-called “war” on illicit drugs, because this control must be achieved without impeding patients’ access to medical care. A rational public policy would attempt to achieve a balance between the need to minimize abuse and the need to provide relief. (shrink)

This book examines the use, principally in economics, of the concept of the invisible hand, centering on Adam Smith. It interprets the concept as ideology, knowledge, and a linguistic phenomenon. It shows how the principal Chicago School interpretation misperceives and distorts what Smith believed on the economic role of government. The essays further show how Smith was silent as to his intended meaning, using the term to set minds at rest; how the claim that the invisible hand is the foundational (...) concept of economics is repudiated by numerous leading economic theorists; that several dozen identities given the invisible hand renders the term ambiguous and inconclusive; that no such thing as an invisible hand exists; and that calling something an invisible hand adds nothing to knowledge. Finally, the essays show that the leading doctrines purporting to claim an invisible hand for the case for capitalism cannot invoke the term but that other nonnormative invisible hand processes are still useful tools. The aim of this paper is to offer a new biological interpretation of Fisher’s ‘Fundamental Theorem of Natural Selection’ and from this to consider optimality properties of gene frequency changes. These matters are of continuing interest to biologists and philosophers alike. In particular, the extent to which biological evolution can be calculated from the ‘gene’s-eye’ point of view is also discussed. In this sense, the paper bears indirectly on the concepts of the unit of selection and of the ‘selfish gene’. (...) Ecological feminism is the position that there are important connections-historical, symbolic, theoretical-between the domination of women and the domination of nonhuman nature. I argue that because the conceptual connections between the dual dominations of women and nature are located in an oppressive patriarchal conceptual framework characterized by a logic of domination, (1) the logic of traditional feminism requires the expansion of feminism to include ecological feminism and (2) ecological feminism provides a framework for developing a distinctively feminist environmental ethic. This text looks at the potential benefits of concept and theory formation along dynamic, evolutionary and valuation for understanding economic processes.

This paper is a commentary on the focal article by Grafen and on earlier papers of his on which many of the results of this focal paper depend. Thus it is in effect a commentary on the “formal Darwinian project”, the focus of this sequence of papers. Several problems with this sequence are raised and discussed. The first of these concerns fitness maximization. It is often claimed in these papers that natural selection leads to a maximization of fitness and that (...) this view is claimed in Fisher’s “fundamental theorem of natural selection”. These claims are refuted, and various incorrect statements about the meaning and interpretation of the fundamental theorem of natural selection, in this sequence and in other papers by other authors, are discussed. Next, much of the work in this sequence rests on the first Price equation. In the deterministic (infinite population) case this equation is no more than the standard classical equation relating to changes in gene frequencies. In the stochastic case the equation gives the change in gene frequencies as the sum of two terms (the second of which vanishes in the deterministic case). These two terms are of essentially equal importance in the situation considered in the focal article, yet one of Grafen’s results ignores the second term in the stochastic analysis. This is associated with a wavering between deterministic and stochastic analyses and the use of the Price fitness concept and the classical fitness concept. These comments cast doubts on Grafen’s optimization theory. (shrink)

There is no such thing as the future of Man. Or, rather, there is only one circumstance in which that phrase has any real meaning, that circumstance being the complete eradication of all humans at more or less the same time or as a result of one event—such as a large enough asteroid strike, a widespread major volcanic episode, or a massive nuclear exchange. Barring such an event, however, different groups of humans, even different groups within individual societies, have different (...) futures. Even if we try to think about the future of the human species in the abstract, there are many different futures that can be conceived, and even these different conceptions do not exhaust the topic, for our thinking about our future and our knowledge are linked—our knowledge shapes our thinking about our futures, and the ideas we have about our futures help direct the knowledge that we set out to gain—which then shapes the new thoughts we have about our futures, which are always unfolding before us. The Pareto principle is in fact the fundamental concept of welfare economics. However, it has serious analytical and heuristic limits, is selective and conservative in nature and use, and is heavily normative notwithstanding the pretensions by advocates of its positive character.

The current feminist debate over ecology raises important and timely issues about the theoretical adequacy of the four leading versions of feminism-liberal feminism, traditional Marxist feminism, radical feminism, and socialist feminism. In this paper I present a minimal condition account of ecological feminism, or ecofeminism. I argue that if eco-feminism is true or at least plausible, then each of the four leading versions of feminism is inadequate, incomplete, or problematic as a theoretical grounding for eco-feminism. I conclude that, if eco-feminism (...) is to be taken seriously, then a transformative feminism is needed that will move us beyond the four familiar feminist frameworks and make an eco-feminist perspective central to feminist theory and practice. Warren Introduction In the Preface to my book, Ecofeminist Philosophy: A Western Perspective on What It Is and Why It Matters, 1 I describe as both "exciting and taxing" the process of writing the book over more than one decade (Warren, x). It was exciting because I was contributing to (...) the still nascent field of ecofeminist philosophy; it was taxing because I often found it difficult to know what I wanted to say about ecofeminism as a philosophical position. The book that emerged is my attempt to say, in my own voice and in a language and style amenable to a reflective lay audience, what I understand ecofeminist philosophy to be and why I think it matters.Ecofeminist Philosophy is premised on the conviction that ecofeminist philosophy has liberating potential to generate insights and recommendations for any theory, practice, or policy that is feminist, ecofeminist, or environmental. The version of ecofeminist philosophy I defend focuses on a variety of contingent interconnections—historical, empirical, socioeconomic, conceptual, linguistic, symbolic and literary, spiritual and religious, epistemological, metaphysical, political, ethical, and theoretical—among the dominations of women, other subordinated humans, nonhuman animals, and nature. It locates the commonalities of mutually reinforcing "isms of domination" (e.g., sexism, racism, classism, heterosexism, ageism, colonialism, [End Page 39] ethnocentrism, speciesism and "naturism," or the unjustified domination of nonhuman nature) in oppressive conceptual frameworks, institutions, and practices mediated and justified by a "logic of domination." Connecting these isms of domination to pressing contemporary feminist and environmental issues, I argue for a version of ecofeminist philosophy that provides both critical analyses of and creative solutions to the intersecting practices, policies, and structures of unjustified ("isms" of) domination.From my perspective, the book is bold and daring. It advocates positions (e.g., regarding spirituality) typically eschewed by feminist and ecofeminist philosophers. It takes positions (e.g., on moral vegetarianism) that are at odds with much mainstream ecofeminist literature on nonhuman animals (particularly ecofeminist animal rights literature). It defends unfamiliar or unpopular philosophical positions (e.g., on the moral considerability of nonhuman animals and nature, on the nature of social justice, on ethics as "care-sensitive," and on theory as theory-in-process). It provides an account of ecofeminist philosophy that is grounded in good science, though not in canonical ideals of objectivity and rationality. It uses cross-cultural empirical, historical, social, and cultural data to motivate and test the theoretical claims of ecofeminist philosophy. Deception has been practiced by sellers since the beginning of the marketplace. Research in marketing ethics has established benchmarks and parameters forethical behavior that include honesty, full disclosure, equity, and fairness. Deception in marketing, however, has not received the same level of attention. This paper proposes to treat deception in marketing within the context of criminology. By examining deception in marketing within the context of criminology, additional insight can be gained into identifying its antecendents and the likelihood of its occurrence. (...) To this end, deception in marketing is interpreted under the empathy/harm matrix, Cressey’s fraud triangle, and the transparency/time-lag matrix. These frameworks are then combined into a diagram detailing antecedents affecting the likelihood of marketing agents participating in deceptive marketing actions.
